Forms
Purpose
The Forms screen is used to create and manage online forms for collecting Lead and Customer information within Thulo Marketing Automation.
Forms can be used for various marketing and lead generation activities, including:

Lead Capture Forms

Newsletter Subscription Forms

Event Registration Forms
-
Asset Download Forms
-
Contact Request Forms

Marketing Campaign Forms
When a visitor submits a form:
-
The submitted information is stored in the Leads module.
-
The Lead is associated with the corresponding Form.
-
The data becomes available for Campaigns, Marketing Automation Workflows, Segments, and Lead Management.
Forms serve as one of the primary entry points for generating Leads in the Marketing Automation system.
Create
Step 1: Create a Form

Navigate to Marketing Automation → Components → Forms.

Click Add.
-
Complete the required form settings.
-
Click Save.
After saving, the system redirects to the Form Builder screen.
Step 2: Design the Form
Once the Form record has been created:

Open the Form Builder tab.
-
Drag and configure the required form fields.
-
Arrange the form layout.
-
Save the form design.
The form is now ready to be published and shared.
View
To view a Form:

Navigate to Marketing Automation → Components → Forms.

Locate the Form.
-
Click View.
The Form details screen displays:

Form configuration

Form Builder
-
Integration Code
-
Submission statistics
-
Associated Leads
Edit
To edit a Form:
-
Navigate to Marketing Automation → Components → Forms.
-
Locate the Form.
-
Click Edit.
-
Update the required settings or form fields.
-
Click Save.
Changes are applied immediately to future submissions.
Delete
To delete a Form:

Navigate to Marketing Automation → Components → Forms.
-
Locate the Form.
-
Click Delete.
-
Confirm the deletion.
The Form is permanently removed from the system.
Field Descriptions
| Field | Description |

| Source | Identifies the marketing source associated with the Lead, such as Google, Facebook, Bing, or custom sources. |
| Status | Lead status assigned when the form is submitted and the Lead record is created. |
| Responsible (Assignee) | Staff member responsible for managing Leads generated through the Form. Data is retrieved from the Staff module. |
| Language | Language assigned to the Form. |
| Submit Button Text | Text displayed on the form submission button. |
| Display Thank You Message | Message displayed after successful form submission. |
| Redirect to Another Website | URL where the visitor is redirected after successful form submission. |
| Notification Settings | Configuration used to send notifications when new form submissions are received. |
| Auto Mark as Public | Automatically marks newly created Leads as public and visible to all staff members. |
| Allow Duplicate Lead to be Inserted into Database | Determines whether duplicate Lead records can be created. |
| Prevent Duplicate on Field | Field used to identify duplicate Leads. |
| Additional Duplicate Check Field | Optional secondary field used for duplicate detection. |
| Create Duplicate Lead Data as Task | Creates a task when duplicate Lead information is submitted. |
| Form Builder | Interface used to create and design form fields. |
| Form Information & Setup | Displays form configuration and setup information. |
| Integration Code | Provides embeddable code and shareable links for the Form. |
Workflow Rules
Lead Creation
When a visitor submits a Form:

The system validates the submission.
-
A Lead record is created or updated.
-
The Lead is stored in the Leads module.
-
The Lead is associated with the Form.
-
The Lead becomes available for Campaigns and Marketing Automation workflows.
Form Lead Association
Each Form maintains its own Lead list.
All Leads who submit a specific Form are automatically included in the corresponding Form's Lead records.
This allows administrators to:

Track form performance
-
Segment Leads by form source
-
Build Campaigns using form-generated Leads
Notification Rules
When Notify When Lead Imported is enabled, notifications can be sent using one of the following methods:
Specific Staff Members
Selected staff members receive notifications whenever a form submission occurs.
Staff Members with Roles
All staff members assigned to the selected role receive notifications.
Responsible Person
The staff member selected in the Responsible (Assignee) field receives the notification.
Duplicate Lead Handling
Allow Duplicate Lead to be Inserted into Database = Enabled

The system allows duplicate Lead records.

Multiple records with the same information may be stored.
Allow Duplicate Lead to be Inserted into Database = Disabled

The system checks for duplicate Lead information.
-
Duplicate records are prevented based on configured fields.
Administrators can configure:

Primary duplicate detection field

Additional duplicate detection field
Duplicate Lead Task Creation
When Create Duplicate Lead Data as Task and Assign to Responsible Staff Member is enabled:
-
The system detects a duplicate Lead.
-
A task is automatically created.
-
The task is assigned to the Responsible staff member.
-
The staff member can review and resolve the duplicate Lead.
When disabled:
-
Duplicate Leads are not saved.
-
No task is created.
Public Lead Rules
When Auto Mark as Public is enabled:
-
Newly created Leads are automatically marked as public.
-
All authorized staff members can view the Lead.
When disabled:
-
Lead visibility follows standard ownership and permission rules.
Form Submission Behavior
After successful submission, the system can perform one of the following actions:
Display Thank You Message
A custom success message is displayed to the visitor.
Redirect to Another Website
The visitor is redirected to the specified URL.
This is commonly used for:

Asset downloads

Landing pages

Event registration confirmations

Thank-you pages
Statistics
Total Submissions
The Total Submissions metric displays the total number of successful form submissions received.
This statistic helps measure:

Form performance

Lead generation effectiveness

The Form Builder tab allows administrators to:

Displays the Form configuration and settings.
Administrators can modify form behavior and submission options from this section.

Administrators can adjust:

Embedded display settings
The generated code can be embedded into websites, landing pages, blogs, or external applications.
Important Notes

Every successful form submission creates or updates a Lead record.
-
Form-generated Leads are automatically available for Campaigns and Marketing Automation workflows.
-
Each Form maintains its own Lead list and submission statistics.

Duplicate Lead handling depends on the configured duplicate detection settings.
-
Notification settings help ensure timely follow-up on new Leads.

Forms can be used for lead generation, asset downloads, newsletter subscriptions, event registrations, and other marketing activities.

Redirect URLs should be tested before publishing the Form.

Integration codes can be embedded on external websites to collect Leads directly into Thulo Marketing Automation.

Total Submission statistics are updated automatically as visitors submit the Form.
